Wolverhampton PCP Service

MacIntyre has been working in partnership with Wolverhampton City Council and the Primary Care Trust to develop a person centred planning service for people with learning disabilities living in the city. This exciting project aims to provide adults who have a learning disability and their families with the opportunity to develop a person centred plan.

Person centred planning uses a variety of tools to enable people to identify what is important to them now, what will be important to them in the future and what support they need to ensure that they are able to achieve meaningful changes and community inclusion.

Key to the success of the Wolverhampton Person Centred Planning service has been its efforts to reach areas of the community that historically have been harder to engage for social care services.

By placing information stands and drop-in sessions in local community facilities such as libraries, the local shopping centre and indoor market, a greater cross-section of the community has been reached.

The project has also used routes such as local neighbourhood partnerships and carers groups. Forging links with special interest groups, such as the ACCI (Afro-Carribean Community Initiative), has proven particularly successful and the project continues to focus strongly on the harder to reach elements of the community with the message of how person centred planning can assist everyone in their day to day lives.
