Create a PCP Plan

Everyone's Person Centred Plan is different and therefore two plans are never the same. See the 6 stage planning process below for an example of Person Centred Planning in action.

1. Choose a Facilitator

The Facilitator plays a key role in the PCP process.

They would usually be a family member, close friend or staff member.

Someone that the individual likes and trusts.
2. Choose a Tool

There are a number of different tools that can be used when approaching PCP.

3. Choose Whom to Involve

It is at the discretion of the individual who they choose to involve in their PCP.

An individual can involve as many or few people as they like, each individual often has a different level of involvement.

4. Get Together to plan
Elvis
Planning does not have to be a formal activity.

It is up to the individual when they choose to plan, at a time and place that is convenient and appropriate to them.
5. Strive to Achieve your Goals

Following a get together or meeting a number of goals are often identified.

The members should positively support the individual in working towards achieving their goals.
6. Reflection-Are you Happy with Outcomes

This is an ongoing process, assessing whether the individual is happy and what they want to work towards achieving next.
