Cymorth yng Nghymru
In Wales we offer Supported Living for people over the age of 16 who have care needs relating to physical disabilities, sensory loss or people with learning disabilities to people living in the Wrexham or Powys area.
We provide person centred, meaningful care to the people we support which is focused on increasing the independence of each individual. The people we support are positively encouraged to make decisions about the care and support they receive, and each individual’s preferences are recognised and valued.

Yng Nghymru rydym yn cynnig byw â chymorth i bobl dros 16 oed sydd ag anghenion gofal sy'n ymwneud ag anableddau corfforol, colli synhwyrau neu bobl ag anableddau dysgu i bobl sy'n byw yn ardal Wrecsam neu Bowys.
Rydym yn darparu gofal sy'n canolbwyntio ar yr unigolyn ac sy'n ystyrlon i'r bobl rydym yn eu cefnogi sy'n canolbwyntio ar gynyddu annibyniaeth pob unigolyn. Mae'r bobl yr ydym yn eu cefnogi yn cael eu hannog yn gadarnhaol i wneud penderfyniadau am y gofal a'r cymorth y maent yn eu derbyn, ac mae dewisiadau pob unigolyn yn cael eu cydnabod a'u gwerthfawrogi.
